Access is restricted to individuals who meet all of the following criteria:

People registered as users of the platform
People who have received training provided by the person in charge of the platform
People who have successfully passed a practical exam
People who made an online reservation

Access to the instruments may be revoked totally or partially for a specific or indefinite period in the event of repeated non-compliance with the access rules

L'accès est réservé aux personnes qui remplissent tous les critères suivants:

Personnes inscrites en tant qu'utilisateur de la plateforme
Personnes ayant reçu une formation dispensée par le responsable de la plateforme
Personnes ayant passé avec succès un examen pratique
Personnes ayant fait une réservation en ligne

L’accès aux instruments peut être révoqué totalement ou partiellement pour une durée déterminée ou indéterminée en cas de non-respects répétés des règles d'accès.

Téléchargez le formulaire Formulaire_Nouvel-Utilisateur.xlsx
Complétez les deux onglets du formulaires (Utilisateur et Facturation)
Enregistrez le formulaire sous la nomenclature suivante : Formulaire_Prénom_Nom.xlsx
Envoyez le formulaire complété à Histologie@cib.umontreal.ca
Après votre inscription vous recevrez un courriel du système de réservation FACES avec un mot de passe temporaire pour le groupe Histologie_UdeM

Download the form New-User_Form.xlsx
Complete the two tabs of the form (User and Billing)
Save the form under the following nomenclature Formulaire_Firstname_Lastname.xlsx
Send the completed form to histologie@cib.umontreal.ca
After registration you will receive an email from the FACES reservation system with a temporary password for the Histologie_UdeM group
